The HealthMatters, Illinois! is a health promotion initiative aimed at improving health and health promotion of people with intellectual and developmental disabilities (I/DD) throughout the state of Illinois. HealthMatters Program team has partnered with Illinois Disability and Health Program at the Illinois Department of Public Health to assist provider organizations with promoting healthy lifestyles to people with I/DD while meeting the requirements of the National Core Indicators.

WHO CAN PARTICIPATE?
We are looking for motivated Organizations who provide Supports for Community Living services to people with I/DD in Illinois to participate in this initiative.
Up to 15 provider organizations will be selected to participate and will receive resources to benefit people with I/DD receiving services, the organization, and their employees. Each organization will need to devote the staff to make up a team that includes but is not limited to: two (or more) staff leaders and a HealthMatters Team Coordinator who will ultimately become Certified Instructors to teach 12-week HealthMatters Program to people with I/DD.

WHAT WILL YOU DO?
Organization needs to provide structured time for a 12-week HealthMatters Program for people with I/DD, staff, and Wellness Committee to meet. This project is one (1) year long and requires the following commitments:
