The HealthMatters, Maryland! is a health promotion initiative aimed at improving health and health promotion of people with intellectual and developmental disabilities (I/DD) throughout the state of Maryland. HealthMatters Program team has partnered with Maryland Department of Disabilities to assist provider organizations with promoting healthy lifestyles to people with I/DD while meeting the requirements of the National Core Indicators.

We are looking for motivated people in Organizations who provide Supports for Community Living services to people with I/DD in Maryland to participate in this initiative.
Up to 10 provider organizations will be selected to participate and will receive resources to benefit people with I/DD receiving services, the organization, and their employees. Each organization will need to devote the staff to make up a team that includes but is not limited to: two (or more) staff leaders and a HealthMatters Team Coordinator who will ultimately become Certified Instructors to teach 12-week HealthMatters Program to people with I/DD.

Organization needs to provide structured time for a 12-week HealthMatters Program for people with I/DD, staff, and Wellness Committee to meet. This project is one (1) year long and requires the following commitments:
Complete online Application and submit Letter of Interest February 17th, 2017
Create a Wellness Committee March/April 2017
Complete organizational HealthMatters Assessments (staff only, at least 25% of workforce, must be completed at least 2 weeks before webinars). April 7th, 2017 (Time 1)
April 6th, 2018 (Time 2)

Create and implement Strategic Plan for health promotion. February 2017-February 2018
Attend three (3) 90-minute HealthMatters Train-the-Trainer Webinars and complete 90 minutes of offline work between webinars (minimum 3 participants). Option 1: April 17, 19, 21 (11am- 12:30pm EST)

Option 2: April 17, 19, 21 (2pm - 3:30pm EST)

Option 3: April 24, 26, 28 (11am- 12:30pm EST)

Option 4: April 24, 26, 28 (2pm - 3:30pm EST)
Run at least one 12-week HealthMatters Program (3 times per week, 4-6 hour per week, minimum 6 participants with I/DD) Start within 2 weeks of the last Webinar.
