The HealthMatters, Nebraska! is a health promotion initiative aimed at improving health and health promotion of people with intellectual and developmental disabilities (IDD) throughout the state of Nebraska. HealthMatters Program team has partnered with Nebraska Total Care to assist provider organizations with promoting healthy lifestyles to people with I/DD

WHO CAN PARTICIPATE?
We are looking for motivated people in DD Provider Organizations who provide services to people with I/DD in Nebraska Total Care network to participate in this initiative.
Up to 6 provider organizations will be selected to participate and will receive resources to benefit people with IDD receiving services, the organization, and their employees. Each organization will need to devote the staff to make up a team that includes but is not limited to: two (or more) staff leaders and a HealthMatters Team Coordinator who will ultimately become Certified Instructors to teach 12-week HealthMatters Program to people with I/DD.

WHAT WILL YOU DO?
Organization needs to provide structured time for a 12-week HealthMatters Program for people with I/DD, staff, and Wellness Committee to meet. This project is one (1) year long and requires the following commitments:
ACTIVITY DEADLINES
Complete online Application and submit Letter of Interest September 8th, 2017
Create a Wellness Committee September/October 2017
Complete organizational HealthMatters Assessments (staff only, at least 25% of workforce, must be completed at least 2 weeks before webinars). Fee for service, $750/organization. Time 1 due October 27th, 2017; Time 2 - TBD

Create and implement Strategic Plan for health promotion. September 2017-September 2018
Attend three (3) 90-minute HealthMatters Train-the-Trainer Webinars and complete 90 minutes of offline work between webinars (minimum 3 participants). January 2018 (time/date TBD)
Run at least one 12-week HealthMatters Program (3 times per week, 4-6 hour per week, minimum 6 participants with I/DD) Start within 2 weeks of the last Webinar.
