HealthAlert: Signs&Symptoms™ Program aims to help people with IDD and their supports to observe (and report) early signs and symptoms of new or changing health concerns for effective communication (Evidence-informed).

Program provides specific instructions for Direct Support Professionals 
(DSPs) or other support persons to do the following:

  1. observe early signs and symptoms of new or changing health concerns among people with IDD,
  2. communicate observations to CBOs nurses, and
  3. implement continuity of health communication between team (DSPs, RNs, PCP, Specialists, Administrators) – support “the voice” of DSPs.

Training Overview

HealthAlert: Signs&Symptoms™ Program is a two hour training program instructs Direct Support Professionals (DSPs) to observe early signs and symptoms of new or changing health concerns among their clients who have I/DD and accurately communicate these observations to nursing personnel in Community-Based Organizations (CBOs). It aims to establish continuity of health care services so that nursing staff and administrators can see DSPs as health advocates and develop better communication strategies in organizations that support “the voice” of DSPs. The training may incorporate the FASSTrack© Software Program (Fast Assessment of Signs and Symptoms Tracking).

Audience

Staff in CBOs working with individuals with I/DD.
Workshop space is limited to 20-25 participants.

Learning Objectives

By the end of the workshop, you will be able to:

  1. Identify the importance of early detection of new or changing health problems in persons with I/DD,
  2. Review the role of DSPs in observing and reporting signs and symptoms of emerging health problems,
  3. Review the importance of observing and reporting emerging health problems among persons with I/DD,
  4. Practice strategies for better communication using the signs and symptoms observation checklist, and
  5. Describe Signs and Symptoms Checklist and how to use it. 

CEUs/CPEUs

Three (3) CEUs/CPEUs provided for Advance Practice Nurses, Registered Nurses, Licensed Practical Nurses, Licensed Clinical Social Workers (LSW, LPC, LCPC), Licensed Dietitian Nutritionists (RD), Nursing Home Administrators, Occupational Therapists, Physical Therapists, Qualified Intellectual Disabilities Professionals (QIDP), Speech Language Pathologists, and Illinois Teachers (CPDUs). Add extra $15 per person for CEUs.
